Diagnosing recurring product and technical delivery problems

Recurring defects, regressions, delays and rework raised a more important question: did the delivery system justify further investment?

Problem → Investigation → Decision

01 / Visible problem

What was happening

Technical, architectural, design and reliability defects resurfaced after fixes. Rework, technical debt and delivery delays grew, while requirements needed constant clarification.

02 / Investigation

What I looked at

I reviewed requirement quality, testing evidence and defect patterns, decision history, development handoffs, technical processes and documentation, ownership and accountability, business-to-technical communication, capability relative to business needs, and where technical knowledge was concentrated.

03 / What changed

The shift

The work moved beyond fixing incidents one by one: clarify requirements and decision records; coordinate product, business and technical delivery; use test evidence to identify regressions and patterns; document key technical decisions and processes; and assess further fixes against reliability, customer risk, capability and opportunity cost.

04 / Result or decision

What the evidence made possible

Evidence about unresolved reliability, delivery capability and customer risk made a public launch difficult to justify. Despite the substantial time and money already invested, the decision was not to proceed to public launch.

Judge whether the underlying delivery system and technical capability warrant further investment, rather than treating sunk costs as a reason to continue.
